How do you make sure a corrective action is effective and not just a temporary fix?

I define an effectiveness criterion before closure. Depending on the issue, I may monitor yield, defect recurrence, process capability, audit evidence, or a defined number of lots. I also check whether the escape control has improved, not only whether the occurrence cause was corrected.
