How do you manage the failure analysis in your work?

In my previous role, I started by reviewing failure reproduction, measurement data, and root-cause evidence. I then worked with the responsible team to confirm symptom, reproduce the failure, isolate the cause, and verify the fix. I documented the result, tracked any remaining risk, and followed up until the failure analysis met the agreed requirement.
