How do you manage the root-cause investigation in your work?

In my previous role, I started by reviewing 8D evidence, verification data, and effectiveness monitoring. I then worked with the responsible team to separate occurrence cause, escape cause, containment, and permanent corrective action. I documented the result, tracked any remaining risk, and followed up until the root-cause investigation met the agreed requirement.
